As an Advanced Practice Clinician (APC) at knownwell, you will work alongside a team of committed physicians, APCs, registered dietitians, registered nurses, and registered pharmacists, to provide comprehensive medical care, with a specific focus on the diagnosis, treatment, and prevention of obesity and obesity-related conditions as appropriate in a primary care setting. This position offers an opportunity to make a significant impact on the lives of patients by helping them improve their overall health.
Schedule: Monday-Friday with flexible scheduling, 32 patient contact hours minimum (+ 8hrs of administrative time). Hybrid model of care. No scheduled evening/weekend responsibilities outside of phone On-Call Location: Midtown Manhattan EMR: Athena On-Call: Phone call only, in a rotation TBD frequency. Call volume – very low 24/7/365 physician back-up
Responsibilities: Practice exceptional medicine within knownwell Sees adults for primary care, often with a focus on metabolic health, or adults for metabolic health only. Share a panel with MD/DO + other APCs. Embodies the knownwell mission and values of patient inclusivity in how you deliver care. Coordinates with the broader care team to ensure the patient has the support they need. Documents as necessary to ensure accurate billing, including review of remote patient monitoring data. Communicates with patients in a timely manner through our portal for questions and concerns that have been pre-screened to necessitate a clinician response. See adults ages 16+ for metabolic health and primary care. Ensure panel is open to any individual, regardless of their current health status and comorbidities. Listen to the aspirations of the patient and ensure care is tailored to the patient’s unique health journey. Provide an environment in which patients feel comfortable and free from judgment. Offer exceptional encouragement as patients reach their goals. Coordinate across the collaborative care team Escalate any concerns to your dedicated site leadership team. Provide clinical guidance to medical assistants and nursing teams as assigned. Stay current with licensure and appropriate re-certifications.
