Description

As a Community Health Worker at Waymark, you will be the frontline presence in the community, connecting with patients to provide social support, advocacy, and navigation.

You will work to improve the health of patients from low-income communities by connecting them with various providers and resources.

Key Responsibilities:

  • Attend a 2-3 week long paid training program.
  • Meet patients in the community, in the home, and in healthcare facilities and conduct a needs assessment.
  • Help patients with health-related social issues like homelessness, substance use, and hunger.
  • Work with other CHWs and staff to create a directory of community resources.
  • Manage relationships with healthcare facility providers and partner with the care delivery team.
  • Continuously expand knowledge of community resources, services, and programs available to members.
  • Accompany members to medical appointments as appropriate.
  • Navigate technology systems to document each patient encounter.
  • Meet patients virtually, by phone or video visit, for conversations as appropriate.
  • Support outreach to individuals eligible for Waymark support.
  • Participate in weekly care team huddles.

Minimum Qualifications:

  • Highly organized and self-motivated to work independently.
  • Sound judgment and ability to quickly analyze situations.
  • Ability to work with a diverse community in an empathetic and professional manner.
  • Friendly, energetic, and enthusiastic personality.
  • Desire to help others.
  • Cultural competency and ability to work with diverse groups.
  • Excellent interpersonal communication skills and active listening abilities.
  • Computer literate with experience using technology for virtual communication and documentation.
  • Comfortable with ambiguity and taking on various tasks.
  • Reside within a commutable distance of Greater Seattle.
  • Travel required within surrounding counties (up to 80%).
  • Current Driver's license and access to an insured vehicle.

Preferred Qualifications:

  • Community Health Worker certification.
  • Long-time resident of the Greater Seattle area and knowledgeable of community resources.
  • Experience conducting home visits and outreach.
  • Experience working with managed care patients.
  • Experience in customer- or client-service roles.
  • Knowledge of Greater Seattle Medicaid populations.

Hourly Rate Range: $26.54 - $30.13
