We are seeking a Patient Referral Specialist to join our team at Appledore Family Med GME in Portsmouth, NH. As a Patient Referral Specialist, you will be responsible for coordinating patient referrals and admissions to ensure seamless transitions between care settings.
In this role, you will work closely with patients, families, and healthcare providers to gather information and coordinate logistics. You will also be responsible for maintaining accurate records and communicating effectively with internal and external stakeholders.
