Care Coordinator

Job Overview

As a Care Coordinator at Pomelo Care, you will support the clinical practice's efficiency by owning key operational and administrative processes.

Responsibilities

• Provide care navigation support for patients to find providers in their network and connect to other care or social services.
• Monitor the member support phone line and answer patient inquiries.
• Conduct eligibility checks for new and existing patients and manage the patient offboarding process.
• Oversee patient scheduling, including prioritization, appointment reminders, and provider schedule optimization.
• Manage referrals to health plan's Case Management team.
• Collaborate with Market Operations Lead to develop new workflows to improve care coordinator efficiency.
• Perform other assigned responsibilities.

Requirements

• 4+ years of healthcare administration experience, preferably in virtual care.
• Familiarity with SDOH platforms and referrals.
• Results-oriented with meticulous attention to detail.
• Ability to work cross-functionally with clinicians and comfort with ambiguity.
• Passion for maternal and child health.

Compensation

The expected salary range is $19.23 to $24.04 an hour ($40,000 to $50,000 if annualized). This role is also eligible for equity, giving you an ownership stake in Pomelo's mission.
