Physical Therapist - Skilled Nursing / Home Health

Sanford Health, the largest rural health system in the United States, is seeking a Physical Therapist to join our team at Good Samaritan St Luke's and St John's. The position is full-time, 35-40 hours per week, covering Skilled Nursing Facilities and surrounding area Home Health clients.

The Physical Therapist will plan, organize, and deliver physical therapy programs to help clients/patients/residents recover and improve their mobility, relieve pain, increase strength, and prevent deformities. Responsibilities include:

• Conducting examinations, evaluations, and interventions for clients/patients/residents affected by injury or disease
• Utilizing tools and techniques to administer physical therapy interventions following safeguards
• Documenting client/patient/resident information for evaluation and establishing therapeutic plans
• Educating clients/patients/residents and their family members about treatment plans/schedules and self-exercises
• Demonstrating knowledge of current research into daily treatment of clients/patients/residents

Requirements:

• Appropriate education level required in accordance with state licensure
• Must be licensed in the state(s) of practice as a Physical Therapist
• Basic Life Support (BLS) for healthcare providers required or within the first 60 days of employment

Benefits:

• Salary range: $37.50 - $62.00
• $20,000 sign-on bonus (eligibility requirements apply)
