Physical Therapist Hourly - Wisconsin Rapids Home Health

Aspirus Wisconsin Rapids Hospital & Clinics is seeking a Physical Therapist to join our Wisconsin Rapids Home Health team.

The Physical Therapist provides physical therapy to patients of all ages, evaluating, establishing treatment plans, and providing or supervising treatment modalities. This role also involves patient and family education, as well as documentation.

Responsibilities:

• Evaluate patients and establish treatment plans and goals
• Provide and/or supervise treatment modalities and procedures
• Incorporate patient and family education
• Document all treatments and patient interactions

Requirements:

• Minimum of a Bachelor's Degree in Physical Therapy from a school accredited by the American Physical Therapy Association (APTA)
• State of Wisconsin or Michigan Physical Therapist license
• Current BLS certification or obtained within 90 days of hire
• Valid Driver's License and automobile liability insurance with $100,000/$300,000 coverage
