Coordinates all case management activities with members to evaluate medical needs and facilitate overall wellness. Develops strategies to address issues and enhance member wellness through integration.
Responsibilities
- Instructs programs and procedures in compliance with network management and clinical coverage policies.
- Strong customer service orientation and problem-solving skills.
- Excellent communication and telephonic skills.
- Motivational interviewing skills and ability to build rapport and trust telephonically.
- Excellence in documentation and compliance with regulatory standards.
- Highly organized, self-driven, and able to manage time effectively.
- Achieve performance metrics.
- Technology proficiency.
- Ability to travel within 125 miles round trip for 1 day in the field.
- Variable work schedule with ability to work 2 days weekly until 9 pm local time.
Requirements
- Current/Active Clinical Licensure (RN).
- Case Management experience.
- Must reside in Louisiana in one of the following parishes: Evangeline, St. Landry, Acadia, Lafayette, St. Martin, Iberia, and Vermilion.
Preferred Qualifications
- Bachelor's and/or Master's Degree.
- CCM Certification.
- Previous experience in a Managed Care Organization.
