Description
We are seeking a Clinical Pharmacist to join our team at The Park Hospital in Nottingham. This is a bank role working on an 'as and when required' basis, supplementing staffing levels during busy periods and providing cover for existing staff in times of annual leave or absence.
The successful candidate will conduct medication reviews for inpatients and outpatients, identify interactions, contraindications, and opportunities for medicines optimisation. They will provide tailored advice to patients on their treatment plans, including side effects, compliance, and lifestyle considerations.
Key responsibilities include:
- Conduct medication reviews and identify areas for improvement
- Provide patient advice and support
- Support safe prescribing through reconciliation, clinical screening, and dose adjustment
- Review and authorise prescriptions in line with clinical guidelines and regulatory requirements
- Promote safe and cost-effective use of medicines
- Collaborate with procurement, stock control, and pharmacy technicians to ensure medicine availability
- Monitor and report medication incidents and near misses
To be considered for this role, applicants should have:
- A degree in Pharmacy (MPharm, BSc, or BPharm)
- GPhC registration as a Pharmacist
- Previous experience in a clinical hospital pharmacy role
- Strong knowledge of therapeutics, clinical guidelines, and pharmaceutical legislation
- Excellent communication and clinical decision-making skills
- Ability to work autonomously and as part of a multidisciplinary team
- Confidence using electronic prescribing and medication administration systems
