Description
We are seeking a compassionate Support Worker to join our team at Tree Top View in Leyland, Lancashire.
Tree Top View provides residential support for six adults, including autistic people and individuals with a learning disability, mental health needs, difficulties with communication, visual impairment, sensory needs, epilepsy, food intolerance and allergies, and cultural needs.
As a Support Worker, you will be part of a team committed to delivering personalised care and support to our residents. You will assist qualified staff with assessing and implementing individual care plans, undertake routine tasks and activities, and provide support with daily living tasks.
Key responsibilities include:
- Providing personal care and support with daily living tasks
- Encouraging and assisting participation in activities and community life
- Supporting service users in achieving their personal outcomes
- Maintaining accurate records, care plans, and daily notes
- Building positive relationships with service users, families, colleagues, and stakeholders
- Upholding safety, security, and confidentiality
- Acting as a key worker for individuals, helping them to set goals and track progress
- Promoting choice, independence, and involvement
- Contributing to continuous improvement by sharing ideas and suggestions
