Join our Costco team as a Remote RN Case Manager, where you'll play a key role in helping members achieve better health outcomes through compassionate, coordinated care.
What You'll Do
- Conduct telephonic and occasional in-person assessments to understand member needs
- Develop and manage personalized care plans to improve health and wellness
- Coordinate care across programs, providers, and resources
- Identify risks and remove barriers to care using clinical judgment
- Engage members through meaningful conversations to support their health goals
- Collaborate with a multidisciplinary team to optimize outcomes
Required Qualifications
- Active, unrestricted multistate RN license in state of residence
- 3+ years of clinical nursing experience
- Strong computer skills (Microsoft Office + web-based systems)
- Ability to work the required schedule
Preferred Qualifications
- Case management experience (integrated model)
- Bilingual (Spanish/English)
Education
- Associates Degree in Nursing required
- Bachelor's in Science & Nursing preferred
