Apply on London Borough of Enfield

Description

This is an exciting opportunity to join the London Borough of Enfield (LBE) as we continue to strengthen and develop our services for vulnerable adults in partnership with our health colleagues.

Working within the Assessment and Prevention Hub in the Older People and Physical Disabilities Service, you will play a key role in promoting independence, supporting recovery, and improving outcomes for residents.

Our multidisciplinary team includes Social Workers, Occupational Therapists, and Social Care Assessors, working collaboratively to deliver person-centred support in line with the Care Act 2014.

The Assessment and Prevention Hub focuses on helping people maintain their independence and wellbeing by preventing, reducing, and delaying the need for long-term care and support.

Key responsibilities include:

  • Carrying out timely strengths-based assessments, reviews, and short-term case management.
  • Developing person-centred support plans to ensure individuals receive the right level of care and intervention.
  • Supporting adults following hospital discharge, periods of illness, rehabilitation, recuperation, or a decline in health at home.
  • Completing assessments and support planning within community settings, step-down beds, flexi flats, residential placements, and post-discharge pathways.
  • Working closely with health professionals, community partners, and families to achieve positive outcomes.

You will:

  • Be a registered Social Worker with Social Work England.
  • Have sound knowledge of the Care Act 2014, safeguarding processes, personal budgets, and Continuing Healthcare.
  • Be confident in completing strengths-based, needs-led assessments and support planning.
  • Be committed to multidisciplinary working and partnership approaches.
  • Be willing to undertake the role of a Trusted Assessor, including assessing for and providing minor aids and adaptations.
  • Work effectively with health services, the voluntary and community sector, and social prescribing services.

This is an excellent opportunity to be part of an innovative and supportive team committed to making a real difference to the lives of Enfield residents.

Benefits of working for Enfield Council include:

  • An excellent pension through the Local Government Pension Scheme (LGPS).
  • Up to 32 days annual leave depending on grade and length of service.
  • A blend of remote and office-based working for most roles.
  • Interest-free season ticket loan repayable over three or ten months.
  • Career development and learning experiences from a range of training courses and learning methods.
  • Employee Assistance Programme to provide advice and counselling services.
