Mental Health Referrals Coordinator

Salary

GBP 47,525 - 54,056 per year

Apply on Camden Council

Description

As a Mental Health Pathway Referrals Co-ordinator, you will play a vital role in helping some of Camden's most vulnerable residents access the housing and support they need to live independently and thrive within their communities.

Working as part of the Pathways Move On Team, you will coordinate referrals into specialist mental health supported housing, working closely with housing, health and support partners to ensure people can move into appropriate accommodation as quickly as possible.

Combining operational delivery with partnership working, you will help develop move-on opportunities, prevent homelessness, and support positive outcomes for residents with severe and enduring mental health needs, making a meaningful difference to people's lives at key moments of change.

This post is exempt from the Rehabilitation of Offenders Act and will require a Basic DBS.

We're looking for someone who shares our ambition to make Camden the best place to live and work and who is passionate about delivering services that put residents first.

You will bring a caring, proactive and collaborative approach, with the ability to build positive relationships across housing, health and support services to achieve the best outcomes for residents.

You'll be confident communicating with people from a wide range of backgrounds, able to manage sensitive situations with empathy, and committed to making services accessible, inclusive and responsive to individual needs.

Comfortable using systems and data to support effective decision-making, you will have strong organisational skills and a commitment to continuous improvement.

Most importantly, you will share Camden's values, champion equality, diversity and inclusion, and be dedicated to helping residents access the support, housing and opportunities they need to live independently and thrive.

At Camden, you'll receive a host of benefits. From flexible working to 27 days annual leave (+ bank holidays!). We also know that a work-life balance is extremely important so we also offer family leave, loan schemes and access to our incredible staff networks.
