Description

We are seeking a compassionate and organised Band 7 Guided Care Matron (CCT) to join our team in Hillingdon.

As a Guided Care Matron, you will work as part of a Care Connection Team (CCT) providing anticipatory care to patients to prevent escalation of their health and social care needs, thereby reducing the risk of emergency hospital admissions.

The service works with patients over the age of 18 years with one or more of the following conditions: COPD, Stroke, Cardiovascular Disease, Dementia, Social Isolation, and Diabetes Mellitus.

Key responsibilities include:

  • Developing care plans for patients on the CCT caseload
  • Proactively identifying at-risk patients
  • Attending weekly huddles with the CCT to discuss patients and formulate care plans
  • Making referrals to key stakeholders such as district nurses, geriatricians, social services, and third-sector organisations
  • Working autonomously to provide a full range of services predominantly to house-bound patients, including care homes and supported living
  • Carrying out holistic assessments
  • Using your own car for domiciliary visits
  • Managerial aspects, including attending key meetings and supervising junior staff

The salary for this position ranges from £55,524 to £62,652 per annum.

Skills

Required

  • RGN: Adult nurse
  • Evidence of relevant Continuous Professional Development
  • Non-Medical Prescribing Qualification or willing to undertake within 12 months of appointment
  • Advanced Assessment Skills/Nurse Practitioner Qualification/Community Matron qualification or willing to undertake within 6 months of appointment
  • BSc or equivalent experience Assessor/Mentor
  • Numeracy skills required for the safe administration of medicines
  • Able to communicate with the multi-disciplinary team
  • Maintaining accurate, timely, concise and legible clinical records
  • Manage others and contribute to the management of the team
  • Contribute to the culture of the team and play a role in making it inclusive, kind and hard working
  • Able to talk to people about sensitive issues in a supportive manner
  • Access to Car or other motor vehicle for use for work purposes
  • Able to work efficiently in the community
  • Full UK licence holder
  • Able and willing to work in partnership with service users, their friends and family as well as other health care professionals

Nice to have

  • Mentorship Qualification
  • Experience of working in A&E, ITU, or other equivalent acute medicine specialty
  • Expertise in particular skills
