Job Summary: The purpose of a Nurse Practitioner at Oak Street Health is to provide effective and equitable value-based primary care to adults on Medicare to keep them happy, healthy, and out of the hospital.
Core Responsibilities:
- Provide exceptional primary care
- Conduct office visits for routine and acute issues
- Administer Annual Wellness Visits and health risk assessments
- Coordinate care with other providers, specialists, testing facilities, and agencies
- Lead population health initiatives
- Assist with phone triage and outreach
- Educate patients on their health conditions, care plans, and treatments
- Participate in promotional activities
- Conduct home visits as needed
Requirements:
- Master's degree in Nursing or Doctor of Nursing Practice
- National certification in Family Nurse Practitioner, Adult-Gerontology Primary Care Nurse Practitioner, Adult Nurse Practitioner, or Gerontological Nurse Practitioner
- Active state Nurse Practitioner license
- Active DEA license
- US Work Authorization
Preferred Qualifications:
- Experience in primary care
- Understanding of HCC documentation, ICD-10 coding, and Health Risk Assessments
- Passion for teamwork and collaboration
- Desire to be part of an innovative model focused on population health
- Bilingual proficiency
Salary: $78,331.00 - $168,714.00 per year
