Description
We are seeking a Clinical Pharmacist to join our team at The Woodlands Hospital in Darlington. This is a bank role working on an 'as and when required' basis, providing cover for existing staff during busy periods or annual leave.
The successful candidate will conduct medication reviews for inpatients and outpatients, identify potential issues, and provide tailored advice on treatment plans. Key responsibilities include:
- Conducting medication reviews to identify interactions, contraindications, and opportunities for medicines optimisation
- Providing advice to patients on treatment plans, side effects, compliance, and lifestyle considerations
- Supporting safe prescribing through reconciliation, clinical screening, and dose adjustment
- Reviewing and authorising prescriptions in line with clinical guidelines and regulatory requirements
- Promoting safe and cost-effective use of medicines
- Collaborating with procurement and pharmacy technicians to ensure medicine availability
- Monitoring and reporting medication incidents and adverse drug reactions
To be considered for this role, applicants should have:
- A degree in Pharmacy (MPharm, BSc, or BPharm)
- GPhC registration as a Pharmacist
- Previous experience in a clinical hospital pharmacy role
- Strong knowledge of therapeutics, clinical guidelines, and pharmaceutical legislation
- Excellent communication and clinical decision-making skills
- Ability to work autonomously and as part of a multidisciplinary team
- Confidence using electronic prescribing and medication administration systems
