Description
We are seeking a Family Medicine Advanced Practice Provider (APP) to join our team in the Salt Lake Valley. As an APP, you will have the opportunity to work in both Family and Internal Medicine Clinics, covering long-term leaves and current provider templates across 25 buildings.

Responsibilities:
- Provide high-quality patient care in both Family and Internal Medicine Clinics
- Cover long-term leaves and current provider templates
- Work in a collaborative environment with other healthcare professionals
Requirements:
- Current state license as a Physician Assistant or Advanced Practice Registered Nurse (APRN)
- Certification as a Physician Assistant or Nurse Practitioner
- Eligible for State Controlled Substance License II through V
- Eligible for Federal DEA Certificate II through V
- BLS Certification for Healthcare Providers
- Other certification as required (e.g. ACLS, PALS, TNCC, NRP)
Preferred Qualifications:
- Two (2) or more years of experience
- Master's degree from an accredited institution

- Hourly range: $57.33 - $88.47
