RN Concierge Member Care Coordinator

We are seeking a full-time RN Concierge Member Care Coordinator to join our team at Intermountain Health Park City Hospital.

The successful candidate will serve as a key liaison between Concierge Program participants, physicians, supporting staff, and referring specialists. You will provide clinical support to patients and providers, manage patient relationships, and contribute to growing overall member participation in Intermountain Concierge Services.

Key Responsibilities:

• Provide patient care and member relations, focusing on member growth and participation
• Enroll new concierge members via clinic/patient contracts and outlined in-clinic procedures
• Build and maintain relationships with program participants through effective communication
• Participate in member retention, contract renewal, and payment collection
• Triage patient calls, schedule appointments, and coordinate referrals to other physicians and specialists
• Perform clinical nurse duties, including assessments, planning, and implementing care
• Collaborate with interdisciplinary teams to develop individualised plans of care
• Participate in quality improvement initiatives

Requirements:

• Nursing degree from an accredited institution
• Licensure as a Registered Nurse in the state of practice
• Current Basic Life Support Certification (BLS) for Healthcare Providers
• Two years of clinical nursing experience
• Clinical expertise in assigned clinical areas
• Working knowledge of clinical processes in ambulatory settings
• Demonstrated communication skills
• Experience mentoring clinical staff
• RNs hired or promoted into this role need to have or obtain their BSN within three years of hire or promotion

Preferred Qualifications:

• Bachelor's Degree in Nursing (BSN) from an accredited institution

Physical Requirements:

• Ongoing need to see and read information, labels, and assess patient needs
• Frequent interactions with patient care providers, patients, and visitors requiring verbal communication and hearing
• Manual dexterity for manipulating complex equipment
• Ability to lift and utilise full range of movement to transfer patients
• Need to walk and assist with patients and obtaining and distributing supplies and equipment
