Position Summary Applies advanced clinical judgement to facilitate appropriate member physical health and behavioral healthcare. Implements strategy to assess and plan care, direct provider collaboration, and coordinate psychosocial wraparound services. Instructs the application of available resources to produce optimal, cost-effective outcomes.

Required Qualifications

• Case Management Experience
• Strong Customer Service Orientation and Problem Solving
• Excellent Communication/Telephonic Skills
• Excellent Motivational Interviewing Skills/ability to build rapport and trust telephonically
• Excellence in Documentation and respecting compliance/regulatory standards
• Highly Organized, remain self-driven without direct supervision
• Manage Time Effectively
• Achieve performance metrics
• Technology Proficiency
• Variable work schedules with ability to work 2 days weekly until 9pm local time
• Master's plus BH Clinical License in good standing with no restrictions required (LPC, LMFT, LCSW)

Preferred Qualifications

• Two years Care Management experience.
• CCM Certification
• Previous Experience in Managed Care Organization

Pay Range

• $54,095.00 - $116,760.00
