Description
We are seeking a Physical Healthcare Nurse to join our team at Priory Hospital Kneesworth House in Royston, Hertfordshire.
As a Physical Healthcare Nurse, you will be part of a compassionate team that is committed to supporting people's wellbeing. You will work in a clinical site and ward areas, ensuring patients receive comprehensive physical healthcare assessments on admission, reviewing relevant medical history and results, and accurately documenting and escalating patient information.
Key responsibilities include:
- Supporting and training nursing staff in areas such as chronic disease management, diabetes, hypertension, COPD, wound care, physical observations and healthcare assessment tools
- Overseeing physical healthcare care plans, prescribing within competency, providing ECG, phlebotomy and wound care services
- Preparing clinical reports and discharge documentation, and liaising with primary care services to coordinate screening, vaccinations and ongoing health reviews
- Leading on infection prevention and control (IPC) audits, medical device management, smoking cessation, health education and public health initiatives
- Developing an annual health promotion programme, maintaining positive professional relationships, participating in clinical supervision and reflective practice, and adhering to Priory Group policies and the NMC Code
