Field Care Manager Registered Nurse

As a Field Care Manager Registered Nurse at Humana, you will play a crucial role in ensuring that patients receive high-quality care in the comfort of their own homes.

Your primary responsibilities will include:

• Identifying and resolving barriers to effective care
• Continuously monitoring patient care through assessments and evaluations to ensure progress towards desired outcomes
• Creating plans of care that cater to the unique needs of each patient
• Communicating effectively with internal and external stakeholders
• Understanding departmental, segmental, and organisational strategies and operating objectives
• Following established guidelines and procedures
• Visiting Medicaid members in their homes, Supportive Living Facilities, and Long-Term Care Facilities, which will involve 75-90% local travel

To be successful in this role, you will need:

• A Registered Nurse (RN) licence in the state of Illinois without disciplinary action
• Clinical nursing experience, preferably in hospital, acute care, or home health settings
• Ability to travel extensively within the state of Illinois (75-90%)
• Knowledge of community health and social service agencies, as well as additional community resources
• Proficiency in using various electronic information applications and software programs, including electronic medical records

We prefer candidates with:

• A Bachelor of Science in Nursing (BSN)
• At least 2 years of experience in in-home case or care management
• Experience working with Medicare/Medicaid members
• Health promotion, coaching, and wellness experience
• Previous managed care experience
• Bilingualism (English and Spanish)
• Certification in Case Management
• Motivational Interviewing Certification

In terms of work arrangements, this is a hybrid role that allows you to work from home but requires occasional onsite work at the market office in Schaumburg, Illinois, as well as extensive field travel to visit members.

Your work schedule will be Monday to Friday, 8am to 5pm CST, with a salary range of $71,100 to $97,800 per year.
