Clinical Social Worker

The Clinical Social Worker utilises clinical expertise to perform psychosocial assessments, develop and implement plans of care, assess crisis situations, and provide family and individual treatment.

This position works collaboratively with patients, patient's significant other, families, healthcare providers, insurers, community resources, and all other involved parties.

Responsibilities:

• Assess patients' medical history, mental health history, and current presentation to understand their needs.
• Organise and prioritise daily work by assessing new, current, and discharging patient needs.
• Diagnose and treat various mental, emotional, and behavioural disorders.
• Perform social work consultation for patients and caregivers.
• Develop a plan of care either individually or with input from the patient, patient family and significant others, physicians, and other members of the healthcare team.
• Modify care plans to reflect changes in patient status.
• Refer patients to other services to improve their well-being.
• Educate, communicate, and coordinate with multiple patient care teams and outside agencies as necessary.
• Provide support and debriefing for staff members following adverse events.
• Act as an advocate on behalf of patients and families.
• Work with the multi-disciplinary team to develop a discharge plan.
• Complete timely and accurate documentation in the medical record.

Requirements:

• Master's Degree in Social Work from an accredited institution.
• Current State Licensure as LCSW or LMFT.
• Computer skills, including email and word processing.
• Excellent written and verbal communication skills.

Preferred Qualifications:

• Experience with patients, families, and treatment teams.

Sign-on Bonus:

• $2500.00 sign-on bonus for external candidates.

Credentialing Process:

• Up to a 60-90 day credentialing process required.

Salary Range:

• $35.60 - $54.93 per hour.
