Description
We are seeking a Full Time Home Health Physical Therapist Case Manager for South East Chicago.
The successful candidate will be responsible for assuring and improving quality of care, care and service coordination, leadership and collaboration, patient relationships, business development and growth, patient education.
Responsibilities:
- Facilitate patient and family rights to receive quality, cost-effective care
- Adapt practice to latest standards according to evidence-based literature
- Participate in quality/process improvement initiatives
- Promote efforts to reduce risk and improve patient safety
- Demonstrate effective communication and conflict resolution skills
- Prioritize and organise patient care to achieve optimal health outcomes
- Initiate referrals and discharge planning for assigned patients
- Develop and maintain restorative relationships with patients and families
- Provide care that exceeds patients' and families' expectations
- Assess patient and family readiness to learn and modify approach as necessary
- Demonstrate behaviours that enhance productivity and understanding of financial and economic conditions
Requirements:
- Master's Degree in Physical Therapy
- Licensed in the State of Illinois (or eligible for licensure)
- Basic Life Support (BLS) for Healthcare Providers certification
- 1 year of clinical experience or strong spectrum of clinical affiliation experience
- Ability to lift up to 35 pounds without assistance
- Proficient use of home-based therapy equipment and assistive devices
