Join Oak Street Health’s NP Fellowship in Adult Primary Care, a 12-month paid fellowship that provides all the necessary skills, support, and learning required to become a highly confident and competent clinician for older adults.
During the program, fellows spend time building their own patient panel, learning from a multitude of preceptors, and engaging in didactic content that aims to build on, not repeat, graduate training.
Responsibilities
- Building a patient panel and providing high-quality care to older adults
- Collaborating with a multidisciplinary team to ensure optimal health outcomes
- Engaging in didactic content and continuous learning
Requirements
- Family, adult, or adult-gerontology primary care nurse practitioners who graduated or will graduate from their programs between May 2025 and December 2026 or Physician Assistant who graduated or will graduate from their programs between May 2025 and November 2026
- Must currently reside in, or be willing to relocate to, the Fellowship program location for at least the next two years
- Have not yet worked as an NP or PA
- Minimum of one year experience, within the last five years, working as an adult RN or minimum one year experience in healthcare (for PAs)
- Minimum GPA of 3.0 in NP/PA program
- Have passed, or in the process of, passing NP/PA board examinations by 12/7/26
- Currently holds or will hold by 12/21/26 an active, non-probationary state NP/PA license
- A genuine passion for adult primary care
- A supportive attitude toward our patient population of older adults

- Language skills are strongly preferred, depending on clinic location

The annual salary for this location is $110,000.
Anticipated Weekly Hours: 40
Time Type: Full time
Pay Range: $87,035.00 - $187,460.00
