Description
We are seeking a compassionate, motivated, and clinically confident practitioner to join our Home Dementia Support Service. As a Clinical Practitioner, you will provide specialist, person-centred clinical support to residents living with dementia, initially focusing on those with severe dementia.
Your key responsibilities will include:
- Delivering specialist clinical care through holistic assessments, personalised care plans, and advice to support residents' wellbeing, dignity, independence, and quality of life.
- Empowering care home teams through practical education, coaching, and clinical guidance to strengthen their knowledge, confidence, and capability in dementia care.
- Coordinating multidisciplinary support with residents, families, care home staff, GPs, community and mental health services, social care, and voluntary organisations.
- Supporting residents and families by helping them understand dementia, navigate health and care services, and participate in decisions about their care.
The successful candidate will have a degree-level qualification in a healthcare profession, be registered with a current statutory regulatory body, and have experience of supporting people living with dementia and complex health needs.
In return, we offer a salary of £51,657 to £58,785 per year, a fixed-term contract for 2 years, and flexible working arrangements.
If you are a compassionate, proactive, and clinically confident practitioner who champions dignity and understands the importance of seeing the person beyond their diagnosis, we would love to hear from you.
