Job Summary: The Nurse Practitioner at Oak Street Health provides effective and equitable value-based primary care to adults on Medicare to keep them happy, healthy, and out of the hospital.
Core Responsibilities:
- Provide exceptional primary care
- Conduct office visits for routine and acute issues
- Administer Annual wellness visits and health risk assessments
- Coordinate care with other providers, specialists, testing facilities, and agencies
- Lead population health initiatives
- Assist with phone triage and outreach
- Educate patients on their health conditions, care plans, and treatments
- Participate in promotional activities
- Conduct home visits as needed
Requirements:
- Master of Science in Nursing, Doctor of Nursing Practice, or NP Certificate Program graduate
- National certification in Family Nurse Practitioner, Adult-Gerontology Primary Care Nurse Practitioner, Adult Nurse Practitioner, or Gerontological Nurse Practitioner
- Active, non-probationary state Nurse Practitioner license
- Active DEA license
- US Work Authorization
Preferred Qualifications:
- Experience in primary care: internal medicine, geriatrics, or family medicine
- Understanding of HCC documentation, ICD-10 Coding, and Health Risk Assessments
- Passion for teamwork and cross-functional collaboration
- Desire to be part of an innovative model focused on empirically-guided population health
- Bilingual proficiency
Salary Range: $87,035.00 - $187,460.00
