Description
Central London Community Health Trust is recruiting an experienced and motivated Band 6 Registered Nurse to join our established Watford and Three Rivers Community Nursing Team as a Deputy Team Lead.
The post is offered on a 12-month fixed-term contract until November 2027, working 30 hours per week.
The role offers an excellent opportunity to co-lead one of our neighbourhood teams, working closely with and supported by experienced Team Leaders.
The position is varied and rewarding, with responsibility for managing a patient caseload. You will assess, plan, implement and evaluate evidence-based care for patients in their own homes, helping to support independence and improve outcomes.
We are particularly interested in candidates with previous community nursing experience.
We are looking for someone who is committed to collaborative working, approachable, and an effective communicator. You will be confident working both independently and as part of a team, and open to new ideas and ways of working.
As this is a community-based role, with travel between patient homes, there is a requirement to hold a full, valid, UK driving licence and have access to a car to use for business purposes (unless you have a disability as defined by the Equality Act 2010).
Main duties of the job

- To be responsible for the team's maintenance of patient and other records.
- To participate in the long-term management of patients within the locality.
- To have continued responsibility for a defined caseload.
- To be responsible for the development and maintenance of competencies within the team.
