Physical Therapist

We are seeking a full-time Physical Therapist to join our team at Wake Forest Baptist Medical Center - Outpatient Rehabilitation: Physical Therapy in Winston Salem, NC.

As a Physical Therapist, you will be responsible for providing high-quality patient care, evaluating patients, developing treatment plans, and monitoring progress.

Responsibilities:

• Provide age-appropriate patient care, demonstrating knowledge of growth and development principles across the lifespan
• Interpret medical records and provider referrals, completing evaluations and establishing patient care plans
• Develop and implement treatment plans, including education, exercise, and modalities
• Monitor patient progress, adjusting treatment plans as needed
• Communicate with patients, caregivers, and healthcare professionals to ensure coordinated care
• Maintain accurate and timely documentation of patient care and treatment
• Participate in performance improvement initiatives and professional development activities

Requirements:

• Physical Therapy License in the state of employment
• Basic Life Support (BLS) certification for Healthcare Providers
• Degree in Physical Therapy from an accredited institution
• Doctor of Physical Therapy (DPT) preferred
• Continuing education requirements as mandated by applicable state professional standards

Schedule:

• Monday - Friday, 8am - 5pm
• 1 half-day Saturday every 8 weeks
