Field-Based Case Manager RN

Join Aetna's Long-Term Services and Supports (LTSS) team in a dynamic, field-based Case Manager role where you can make a meaningful difference in the lives of members throughout Virginia.

As a Case Manager, you will take the lead in assessing, planning, and coordinating comprehensive care for members with diverse medical and behavioral health needs.

Key Responsibilities:

• Conducting in-depth, face-to-face assessments to identify member needs
• Developing and implementing individualized care plans
• Coordinating medical, behavioral, and community-based services
• Monitoring progress and adjusting care strategies to improve outcomes
• Advocating for members to ensure access to high-quality, appropriate care

Requirements:

• Current unrestricted Virginia state licensure as a Registered Nurse
• 3 years of clinical experience, preferably in Medical-Surgical Nursing, Behavioral Health, or Long-term Care
• 1+ year of Managed Care experience
• Reliable transportation and willingness to travel up to 75% of the time
• Proficiency with computer skills, including navigating multiple systems and keyboarding

Preferred Qualifications:

• Case Management experience in an integrated model
• 5 years of clinical practice experience
• Effective communication skills, both verbal and written
• Bilingual in English/Spanish
• LTSS experience

Pay Range: $60,522.00 - $129,615.00
