Case Manager

This is a work-from-home position with 50-75% travel to meet members face-to-face. You must live in South Cook or Will County, IL, particularly in zip codes 60461, 60443, 60471, or 60466.

Job Summary This role provides comprehensive healthcare management services to facilitate appropriate healthcare treatment, effectively manage healthcare costs, and improve healthcare program/operational efficiency involving clinical issues.

Key Responsibilities

• Determine medical necessity and appropriateness
• Facilitate optimal outcomes
• Identify and follow through with continuous quality and compliance opportunities
• Educate and empower customers to ensure compliance, satisfaction, and promote patient advocacy
• Optimize total costs through implementation and evaluation of policy based on usage and program directives
• Educate and empower colleagues at all levels to enable decision-making at the most appropriate level

Requirements

• Minimum 2 years of experience in behavioral health, social services, or human services field
• Minimum 2 years of case management experience
• Must reside in South Cook County or Will County, IL, or surrounding areas
• Willing and able to travel using your own vehicle up to 50-75% of the time to meet members face-to-face in South Cook County and surrounding areas
• Reliable transportation required, eligible for mileage reimbursement as per company policy
• Valid Illinois Driver's License

Preferred Qualifications

• Case management and discharge planning experience preferred
• Managed Care experience preferred
• Microsoft Office experience preferred, specifically in Excel

Education

• Bachelor's degree or non-licensed master's level clinician required, with either degree being in behavioral health, social services, or human services field

Pay Range $21.10 - $44.99
