Description
We are looking for a proactive nurse to join our Hospital at Home Case Finders team part-time.
The post holder will assess patient suitability for referral into community services such as Hospital at Home services and intermediate care services.
Responsibilities:
- Identify, assess, and refer patients, keeping a full log and managing communications between teams.
- Work closely with the Integrated Discharge team to ensure all care and services are in place upon discharge.
- Deliver a high-quality service to patients, adapting to meet changing needs.
- Join a roster system to support service cover for 7 days a week, including bank holidays and weekends.
Main duties of the job:
- Identify and case-find potential intermediate care service and/or Hospital at home patients.
- Undertake activities related to clinical, physical, functional, social, and psychological assessment of potential patients.
- Make judgements involving complex facts, requiring interpretation and analysis.
- Prevent premature admission to long-term care through creative problem-solving.
- Promote a seamless and multidisciplinary approach to health and social care provision.

- Registered Nurse
- Further professional studies
- Mentorship course or equivalent
- Experience of negotiating and liaising within a multidisciplinary team
- Experience of working with patients with complex needs
