Description

We are seeking a Physical Therapist to join our team on a floating basis between Advocate Oak Lawn and Advocate Trinity sites.

As a Physical Therapist, you will provide outpatient physical therapy services to patients of various ages and conditions.

Major Responsibilities:

Provide age-appropriate patient care, demonstrating knowledge of growth and development principles across the lifespan
Evaluate patients, develop care plans, and implement evidence-based interventions
Establish functional goals with patients and caregivers, monitor outcomes, and adjust plans as needed
Educate patients and caregivers on diagnoses, treatment plans, and home exercises
Communicate effectively with healthcare providers, staff, and external customers
Complete accurate and timely documentation, maintain productivity standards, and adhere to billing and compliance policies
Participate in performance improvement initiatives, professional development, and continuing education

Requirements:

Physical Therapy license in the state of employment
Basic Life Support (BLS) certification for Healthcare Providers
Doctor of Physical Therapy (DPT) preferred or equivalent educational requirements
Intermediate computer skills and excellent communication, interpersonal, and organizational skills

Physical Requirements and Working Conditions:

Ability to sit, stand, walk, lift, and squat throughout the workday
Lift up to 50 lbs. from floor to waist, up to 10 lbs. from waist to overhead, and carry up to 40 lbs. at waist height
Push/pull with 30 lbs. of force and perform a sliding of 150 lbs. with a second person present
Repetitive use of hands for simple grasping, pushing/pulling, and fine manipulation
