Physical Therapist

We are seeking a full-time Physical Therapist to join our team at Advocate Condell Medical Center - Outpatient Rehabilitation in Gurnee. As a Physical Therapist, you will interpret physician referrals and complete evaluations utilizing appropriate methods. You will establish a plan of care, implement individual treatment, monitor and evaluate outcomes, and adjust the treatment plan as needed. Responsibilities include:

• Interpreting physician referrals and completing musculoskeletal, neuromuscular, cardiopulmonary, and/or integumentary evaluations
• Establishing a plan of care and implementing individual treatment based on evaluation results and functional goals
• Monitoring and evaluating outcomes and adjusting the treatment plan as needed
• Identifying factors affecting patient's physical and psychosocial comfort and attending to these needs
• Planning for and completing patient discharge, including providing written home programs and patient/family teaching
• Communicating and collaborating with stakeholders to achieve optimum care for the patient

Requirements:

• Doctorate degree in Physical Therapy from an accredited school
• Licensed Physical Therapist in the state where you work
• Basic Life Support (BLS) for Healthcare Providers certification
• Intermediate computer skills
• Excellent communication, interpersonal, and organization skills

Physical Requirements and Working Conditions:

• Must be able to sit, stand, walk, lift, and squat throughout the workday
• Must be able to lift up to 50 lbs. from floor to waist, up to 10 lbs. from waist to overhead, and carry up to 40 lbs. at waist height
• Must be able to push/pull with 30 lbs. of force and perform a sliding transfer of 150 lbs. with a second person present
