Description
We are seeking a Clinical Pharmacist to join our team at Woodlands Hospital in Darlington. This is a part-time role for 22.5 hours a week, with a flexible shift pattern.
As a Clinical Pharmacist, you will conduct medication reviews for inpatients and outpatients, identify interactions, contraindications, and opportunities for medicines optimisation. You will provide tailored advice to patients on their treatment plans, including side effects, compliance, and lifestyle considerations.
Key responsibilities include:
- Conduct medication reviews and identify areas for improvement
- Provide patient advice and support safe prescribing
- Review and authorise prescriptions in line with clinical guidelines
- Promote safe and cost-effective use of medicines
- Collaborate with procurement and pharmacy technicians to ensure medicine availability
- Monitor and report medication incidents and near misses
To be considered for this role, you should have:
- A degree in pharmacy (MPharm, BSc or BPharm)
- GPhC registration as a Pharmacist
- Previous experience in a clinical hospital pharmacy role
- Strong knowledge of therapeutics, clinical guidelines, and pharmaceutical legislation
- Excellent communication and clinical decision-making skills
- Ability to work autonomously and as part of a multidisciplinary team
- Confident using electronic prescribing and medication administration systems (e.g., EPMA)
