Physical Therapist (PT) - Outpatient

We are seeking a skilled Physical Therapist (PT) to join our Outpatient Rehabilitation team in Sheboygan, WI. As a Physical Therapist, you will play a crucial role in interpreting physician referrals, completing evaluations, and establishing plans of care for patients. You will work closely with patients, families, and healthcare teams to achieve optimal care outcomes.

Responsibilities:

• Interpret physician referrals and complete musculoskeletal, neuromuscular, cardiopulmonary, and integumentary evaluations
• Establish plans of care and implement individual treatment based on evaluation results and functional goals
• Monitor and evaluate patient outcomes, adjusting treatment plans as needed
• Collaborate with stakeholders to achieve optimum care for patients
• Complete required documentation, including patient evaluations, progress notes, and discharge summaries
• Manage assigned caseload, inform site leadership of coverage needs, and assist with coverage as needed
• Participate in performance improvement activities and pursue professional growth through continuing education

Requirements:

• Doctorate degree in Physical Therapy from an accredited school
• Licensed Physical Therapist in the state of practice
• Basic Life Support (BLS) certification
• Intermediate computer skills and excellent communication, interpersonal, and organization skills
• Ability to lift up to 50 lbs., stand, walk, and sit throughout the workday
