Description

We are seeking a Physical Therapist (PT) to join our Home Health team as a Case Manager on the Chicago South Side. As a Home Health Physical Therapist, you'll manage your own caseload while being supported by a highly collaborative, integrated care team. This role offers autonomy, meaningful patient relationships, and the ability to make a direct impact on patient independence and quality of life in the home setting.

Responsibilities:

  • Provide comprehensive physical therapy assessments and develop individualized care plans
  • Deliver in-home therapy focused on improving mobility, safety, and independence
  • Serve as a case manager, coordinating care with physicians and interdisciplinary teams
  • Educate patients and families to support recovery and long-term outcomes
  • Travel throughout the assigned North Side Chicago territory

Requirements:

  • Licensed Physical Therapist in Illinois (or eligible)
  • Minimum 1 year of clinical experience (or strong clinical affiliation experience)
  • Experience with home-based therapy equipment and assistive devices
  • Strong communication, documentation, and time management skills
  • Ability to work independently while collaborating with a broader care team

Compensation & Benefits:
