Physical Therapist Outpatient

We are seeking a Physical Therapist Outpatient to join our team at Atrium Health Navicent The Medical Center in Macon, GA. As a Physical Therapist Outpatient, you will play a crucial role in providing high-quality patient care and contributing to the success of our organisation.

Job Summary: The Physical Therapist Outpatient will be responsible for performing patient assessments, creating effective treatment plans, and reassessing patients at appropriate intervals. You will also be involved in care planning, interdisciplinary communication, and patient education.

Responsibilities:

• Perform patient assessments and reassessments to develop and modify treatment plans
• Provide care in accordance with physician's orders and ensure quality of care
• Communicate effectively with physicians, nurses, case managers, and other healthcare professionals
• Assess and document patient outcomes criteria
• Demonstrate knowledge of physical therapy modalities and operate equipment safely
• Formulate teaching plans and evaluate effectiveness of learning
• Ensure accurate documentation and charge capture

Requirements:

• Georgia license in physical therapy
• Bachelor's, master's, or doctoral degree in physical therapy
• Good interpersonal and communication skills, time management skills, and flexibility
• Ability to lift 25-60 lbs. and carry 25-50 pounds
