Physical Therapist- Outpatient

We are seeking a skilled Physical Therapist to join our outpatient team in Oswego, IL. As a Physical Therapist, you will play a crucial role in interpreting physician referrals, completing evaluations, and establishing plans of care for patients. You will work closely with a multidisciplinary team to provide high-quality patient care and promote optimal health outcomes.

Responsibilities:

• Interpret physician referrals and complete musculoskeletal, neuromuscular, cardiopulmonary, and integumentary evaluations
• Establish plans of care and implement individual treatment based on evaluation results and functional goals
• Monitor and evaluate patient outcomes, adjusting treatment plans as needed
• Collaborate with healthcare professionals to achieve optimum patient care
• Complete required documentation and maintain accurate records
• Participate in performance improvement activities and contribute to quality of care initiatives

Requirements:

• Doctorate degree in Physical Therapy from an accredited school
• Licensed Physical Therapist in the state of Illinois
• Previous experience in hand therapy required
• Certified hand therapist preferred
• Intermediate computer skills and excellent communication, interpersonal, and organizational skills

Physical Requirements:

• Ability to sit, stand, walk, lift, and squat throughout the workday
• Lift up to 50 lbs. from floor to waist, up to 10 lbs. from waist to overhead, and carry up to 40 lbs. at waist height
• Push/pull with 30 lbs. of force and perform a sliding transfer of 150 lbs. with a second person present
