Description
We are seeking a skilled Physical Therapist to join our outpatient team in Oswego, IL. As a Physical Therapist, you will play a crucial role in interpreting physician referrals, completing evaluations, and establishing plans of care for patients. You will work closely with a multidisciplinary team to provide high-quality patient care and promote optimal health outcomes.
Responsibilities:
- Interpret physician referrals and complete musculoskeletal, neuromuscular, cardiopulmonary, and integumentary evaluations
- Establish plans of care and implement individual treatment based on evaluation results and functional goals
- Monitor and evaluate patient outcomes, adjusting treatment plans as needed
- Collaborate with healthcare professionals to achieve optimum patient care
- Complete required documentation and maintain accurate records
- Participate in performance improvement activities and contribute to quality of care initiatives
Requirements:
- Doctorate degree in Physical Therapy from an accredited school
- Licensed Physical Therapist in the state of Illinois
- Previous experience in hand therapy required
- Certified hand therapist preferred
- Intermediate computer skills and excellent communication, interpersonal, and organizational skills

Physical Requirements:
- Ability to sit, stand, walk, lift, and squat throughout the workday
- Lift up to 50 lbs. from floor to waist, up to 10 lbs. from waist to overhead, and carry up to 40 lbs. at waist height
- Push/pull with 30 lbs. of force and perform a sliding transfer of 150 lbs. with a second person present
