Physical Therapist (PT) - Outpatient

We are seeking a Physical Therapist (PT) for our Outpatient department at Advocate Health. As a Physical Therapist, you will interpret physician referrals and complete evaluations using appropriate methods. You will establish a plan of care, implement individual treatment, monitor outcomes, and adjust the treatment plan as needed.

Responsibilities:

• Interpret physician referrals and complete evaluations
• Establish a plan of care and implement individual treatment
• Monitor and evaluate outcomes, adjusting the treatment plan as needed
• Provide written home programs, patient/family teaching, and referral for community services
• Communicate and collaborate with stakeholders to achieve optimum care
• Complete required documentation and maintain productivity standards
• Participate in performance improvement activities and pursue professional growth

Requirements:

• Doctorate degree in Physical Therapy from an accredited school
• Licensed Physical Therapist in the state
• Basic Life Support (BLS) certification
• Intermediate computer skills and excellent communication, interpersonal, and organization skills
