Physical Therapist (PT) - Home Health-Far South Suburbs

We are seeking a Physical Therapist (PT) to join our Home Health team in the Far South Suburbs. As a Physical Therapist with Advocate at Home, you will have the opportunity to work independently, assessing and addressing patients' mobility, safety, communication, and equipment needs.

Responsibilities:

• Assess, plan, implement, and evaluate physical therapy treatment programs for home care patients.
• Travel extensively within designated geographical boundaries.
• Work rotating weekends and holidays.
• Provide treatment on standard beds in home environments.
• Lift patients using standard pivot transfers or Hoyer lifts.

Requirements:

• Licensed Physical Therapist in Illinois (or eligible for licensure).
• 1 year of Physical Therapy clinical experience.
• Proficient use of home-based therapy equipment.
• Basic computer skills (Microsoft Word, email).
• Strong documentation and interpersonal skills.
