Physical Therapist- FLOAT

We are seeking a Physical Therapist to join our team at Advocate Condell Medical Center. As a Physical Therapist, you will play a crucial role in interpreting physician referrals, completing evaluations, and establishing plans of care for patients. You will also collaborate with other healthcare professionals to achieve optimum care outcomes.

Responsibilities:

• Interpret physician referrals and complete musculoskeletal, neuromuscular, cardiopulmonary, and integumentary evaluations
• Establish a plan of care and implement individual treatment based on evaluation results and functional goals
• Monitor and evaluate outcomes, adjusting the treatment plan as needed
• Communicate and collaborate with stakeholders to achieve optimum care for patients
• Complete all required documentation, including patient evaluations, progress notes, and discharge summaries
• Participate in performance improvement activities and pursue professional growth through continuing education

Requirements:

• Doctorate degree in Physical Therapy from an accredited school
• Licensed Physical Therapist in the state where you work
• Basic Life Support (BLS) for Healthcare Providers certification
• Intermediate computer skills and excellent communication, interpersonal, and organization skills

Benefits:

• Competitive salary range: $41.10 - $61.65 per hour
• $2500 sign-on bonus
• Part-time schedule with 20 hours per week
