Waymark is seeking a Pharmacy Student Clinical Support Specialist to join their team in Ohio. As a Pharmacy Student Clinical Support Specialist, you will play a crucial role in supporting the care team by reviewing patient medical records, summarising medication information, and identifying transitions of care needs.
Key Responsibilities:
- Review patient medical records to gather and summarise medication information
- Identify and document active ambulatory care providers and specialists
- Summarise recent Admission, Discharge, and Transfer (ADT) events
- Extract and identify patient chronic conditions and key provider recommendations
- Flag medical conditions or social determinants of health that may be effectively impacted by the interdisciplinary team
- Provide clear, concise written summaries of findings to direct patient care
Minimum Qualifications:
- Completion of at least 2 years (P2) of an ACPE-accredited Doctor of Pharmacy (PharmD) program or equivalent
- Familiarity with commonly prescribed medications
- Familiarity with traditional management and pharmacological treatment of common chronic conditions
- Proficiency in navigating complex database structures and maintaining HIPAA compliance in a remote work environment
- Strong clinical writing skills
- Ability to commit to 12 hours per week throughout the academic year
- Prior experience navigating Epic Electronic Health Record systems
Preferred Qualifications:
- Completion of 3 years (P3) of an ACPE-accredited Doctor of Pharmacy (PharmD) program or equivalent
- Proven ability to spot discrepancies in medication lists and transition-of-care documentation
- Experience with medication histories
- Experience serving vulnerable populations
The role is remote and offers flexibility, with a regular weekly commitment of at least 12 hours. The expected duration of the role is 6-8 months.
