Description

We are seeking a Pediatric Physical Therapist to join our team at Advocate Condell Medical Center - Outpatient Rehabilitation. As a Pediatric Physical Therapist, you will work on a per diem basis, with a minimum of 8 hours per week, and provide high-quality patient care to pediatric patients.

Responsibilities:

  • Interpret physician referrals and complete musculoskeletal, neuromuscular, cardiopulmonary, and/or integumentary evaluations
  • Establish a plan of care and implement individual treatment based on evaluation results and functional goals
  • Monitor and evaluate outcomes and adjust the individualized treatment plan as needed
  • Provide written home programs, patient/family teaching, and referral for community services
  • Communicate and collaborate with stakeholders to achieve optimum care for patients
  • Complete all required documentation and maintain site/department established productivity standards

Requirements:

  • Doctorate degree in Physical Therapy from an accredited school
  • Licensed Physical Therapist in the State of Illinois
  • Basic Life Support (BLS) for Healthcare Providers certification
  • At least 1 year of experience as a Pediatric Physical Therapist
  • Intermediate computer skills and excellent communication, interpersonal, and organization skills

Physical Requirements and Working Conditions:

  • Ability to sit, stand, walk, lift, and squat throughout the workday
  • Lift up to 50 lbs. from floor to waist and up to 10 lbs. from waist to overhead
  • Push/pull with 30 lbs. of force and perform a sliding transfer of 150 lbs. with a second person present
