Home Health Physical Therapist - Round Lake

We are seeking a Full-time Home Health Physical Therapist to join our team in Round Lake and surrounding communities. As a Physical Therapist, you will play a crucial role in assuring and improving the quality of care for our patients.

Responsibilities:

• Assess patients and develop plans of care to achieve optimal health outcomes
• Provide high-quality, patient-centered care, exceeding expectations and promoting patient/family knowledge and confidence
• Collaborate with interdisciplinary teams to formulate integrated approaches to care
• Participate in quality/process improvement initiatives and promote efforts to reduce risk and improve patient safety
• Develop and maintain a restorative relationship with patients throughout the healthcare continuum

Requirements:

• Doctorate degree in Physical Therapy from an accredited school
• 1 year of clinical experience or strong spectrum of clinical affiliation experience
• Physical Therapy License in the State of Illinois or Wisconsin
• Basic Life Support (BLS) for Healthcare Providers
• Valid driver's license
• Ability to lift up to 35 pounds and work in various environments, including patient homes and assisted living facilities
