Description

We are seeking a full-time Physical Therapist to join our team at Advocate Health in Menomonee Falls, Wisconsin.

The Physical Therapist will work 40 hours per week, with a preferable schedule of four 10-hour days. The caseload will be part orthopedic, but 25% or more of the caseload could be neuro/vestibular. The position requires acute care weekend rotation at the Aurora Medical Center - Washington County, approximately every 6 weeks, and one holiday per year.

Major Responsibilities:

  • Interpret physician referrals and complete musculoskeletal, neuromuscular, cardiopulmonary, and/or integumentary evaluations
  • Establish a plan of care and implement individual treatment based on evaluation results and functional goals
  • Monitor and evaluate outcomes, adjusting the individualized treatment plan as needed
  • Plan for and complete patient discharge, including providing written home programs and patient/family teaching
  • Complete all required documentation, including patient progress notes and discharge summaries
  • Participate in performance improvement activities and manage assigned caseload

Requirements:

  • Physical Therapist license issued by the state of Wisconsin
  • Basic Life Support (BLS) for Healthcare Providers certification issued by the American Heart Association (AHA)
  • Bachelor's Degree in Physical Therapy
  • Intermediate computer skills, excellent communication and interpersonal skills, and demonstrated organization, decision-making, and problem-solving skills

Physical Requirements and Working Conditions:

  • Ability to sit, stand, walk, lift, and squat throughout the workday
  • Lift up to 50 lbs. from floor to waist, up to 10 lbs. from waist to overhead, and carry up to 40 lbs. at waist height
  • Push/pull with 30 lbs. of force and perform a sliding transfer of 150 lbs. with a second person present
