Physical Therapist - Part time

We are seeking a part-time Physical Therapist to join our team at Advocate Sherman Hospital. As a Physical Therapist, you will play a crucial role in interpreting physician referrals, completing evaluations, and establishing plans of care for patients. You will also be responsible for monitoring patient outcomes, adjusting treatment plans, and communicating with physicians and other healthcare professionals.

Responsibilities:

• Interpret physician referrals and complete musculoskeletal, neuromuscular, cardiopulmonary, and integumentary evaluations
• Establish a plan of care and implement individual treatment based on evaluation results and functional goals
• Monitor and evaluate patient outcomes, adjusting treatment plans as needed
• Communicate and collaborate with physicians, patients, and families
• Complete required documentation, including patient evaluations, progress notes, and discharge summaries
• Participate in performance improvement activities and pursue professional growth through continuing education

Requirements:

• Doctorate degree in Physical Therapy from an accredited school
• Licensed Physical Therapist in the state where you work
• Basic Life Support (BLS) certification
• Intermediate computer skills and excellent communication, interpersonal, and organization skills
