Physical Therapist Outpatient

We are seeking a Physical Therapist Outpatient to join our team at Advocate Health in Lake Villa, IL. As a Physical Therapist, you will interpret physician referrals and complete evaluations using appropriate methods. You will establish a plan of care, implement individual treatment, monitor outcomes, and adjust the treatment plan as needed. Responsibilities include communicating with patients, families, and healthcare teams, as well as providing education and training.

Responsibilities:

• Interpret physician referrals and complete musculoskeletal, neuromuscular, cardiopulmonary, and integumentary evaluations
• Establish a plan of care and implement individual treatment based on evaluation results and functional goals
• Monitor and evaluate outcomes, adjusting the treatment plan as needed
• Provide status updates to physicians and other teammates
• Identify factors affecting patient comfort and attend to these needs
• Plan for and complete patient discharge, including providing written home programs and patient/family teaching
• Communicate and collaborate with stakeholders to achieve optimum care
• Complete required documentation, including patient evaluations, progress notes, and discharge summaries
• Maintain site/department established productivity standards
• Apply insurance guidelines regarding authorization and number of visits

Requirements:

• Doctorate degree in Physical Therapy from an accredited school
• Licensed Physical Therapist in the state where you work
• Basic Life Support (BLS) for Healthcare Providers certification
• Intermediate computer skills
• Excellent communication, interpersonal, and organization skills
• Demonstrated organization, decision-making, and problem-solving skills

Physical Requirements and Working Conditions:

• Ability to sit, stand, walk, lift, and squat throughout the workday
• Lift up to 50 lbs. from floor to waist, up to 10 lbs. from waist to overhead, and carry up to 40 lbs. at waist height
• Push/pull with 30 lbs. of force and perform a sliding transfer of 150 lbs. with a second person present
• Repetitive use of hands for simple grasping and fine manipulation
• Exposure to electrical and chemical hazards, blood and body fluids, and road and weather hazards
