Physical Therapist- Outpatient Rehab

We are seeking a Physical Therapist to join our Outpatient Rehab team in Lakemoor, IL. As a Physical Therapist, you will play a crucial role in interpreting physician referrals, completing evaluations, and establishing plans of care for patients. You will also be responsible for monitoring and evaluating patient outcomes, adjusting treatment plans as needed, and communicating with physicians and other healthcare professionals.

Responsibilities:

• Interpret physician referrals and complete musculoskeletal, neuromuscular, cardiopulmonary, and/or integumentary evaluations
• Establish a plan of care and implement individual treatment based on evaluation results and functional goals
• Monitor and evaluate patient outcomes and adjust treatment plans as needed
• Communicate and collaborate with physicians and other healthcare professionals to achieve optimum patient care
• Complete all required documentation, including patient evaluations, progress notes, and discharge summaries
• Participate in performance improvement activities to identify and resolve problems affecting patient care outcomes

Requirements:

• Doctorate degree in Physical Therapy from an accredited school
• Licensed Physical Therapist in the state of Illinois
• Basic Life Support (BLS) certification
• Intermediate computer skills and excellent communication, interpersonal, and organization skills
• Ability to lift up to 50 pounds, stand, walk, and sit for extended periods

Preferred Qualifications:

• Spanish-speaking clinician highly preferred
