Description

We are seeking a Physical Therapist (PT) to join our Home Health team in the South Suburban Chicago communities. As a Home Health Case Manager, you will establish meaningful relationships with patients and families, assessing and addressing their mobility, safety, communication, and durable medical equipment needs.

Responsibilities:

  • Assess, plan, implement, and evaluate physical therapy treatment programs for home health patients
  • Facilitate patient progression toward optimal levels of self-care and functional independence
  • Travel extensively within assigned geographic boundaries in all weather conditions
  • Work rotating weekends and holidays based on patient needs and assigned workload
  • Attend mandatory informational, educational, and branch meetings

Requirements:

  • Licensed Physical Therapist in the State of Illinois (or eligible for licensure)
  • Minimum of one (1) year of Physical Therapy clinical experience, or completion of diverse and comprehensive clinical affiliations
  • Proficiency in the use of home-based therapy equipment and assistive devices
  • Basic computer skills, including Microsoft Word and email; ability to learn electronic scheduling and documentation systems
  • Strong verbal, written, and documentation skills
  • Excellent interpersonal, time management, and self-organization skills

Physical Requirements:

  • Ability to lift up to 35 pounds without assistance
  • Full range of body motion, including bending, sitting, and standing
  • Climbing stairs while carrying objects
  • Pushing, pulling, reaching, lifting, and carrying a minimum of 25 pounds
