Physical Therapist (PT) Case Manager, Home Health

As a Home Health Physical Therapist, you will deliver high-quality, patient-centered care in a home setting, helping patients improve mobility, function, and quality of life.

You’ll collaborate with an interdisciplinary team while working independently across your assigned territory.

Key Responsibilities

• Evaluate, develop, and implement individualized treatment plans based on patient needs and evidence-based practices
• Monitor patient progress and adjust care plans to achieve optimal outcomes
• Provide clear, timely, and compliant documentation to support care and reimbursement
• Educate patients and families on treatment plans, self-care, and safety practices
• Coordinate care with physicians, nurses, and other healthcare team members
• Participate in case conferences, team meetings, and quality improvement initiatives
• Support safe patient handling and promote fall and injury prevention

Requirements

• Licensed Physical Therapist in Illinois (or eligible for licensure)
• Minimum 1 year of clinical experience (or strong clinical affiliation experience)
• Basic computer skills (documentation systems, email, etc.)
• Valid driver’s license and reliable transportation

Work Environment & Physical Demands

• Travel required within assigned service territory
• Work in patient homes and assisted living settings
• Ability to lift up to 35 lbs (with assistance/equipment as needed)
• Frequent standing, walking, bending, and assisting patients with mobility and transfers

• Competitive salary: $44.15 - $66.25 per hour
