Physical Therapist (PT) - Outpatient

We are seeking a Physical Therapist (PT) for our outpatient clinic in Lake Geneva, WI. As a Physical Therapist, you will interpret physician referrals, complete evaluations, establish plans of care, and implement individual treatments. You will also monitor and evaluate patient outcomes, adjust treatment plans, and communicate with physicians and other healthcare professionals.

Responsibilities:

• Interpret physician referrals and complete evaluations
• Establish plans of care and implement individual treatments
• Monitor and evaluate patient outcomes and adjust treatment plans
• Communicate with physicians and other healthcare professionals
• Complete required documentation and maintain accurate records
• Participate in performance improvement activities and contribute to quality of care

Requirements:

• Doctorate degree in Physical Therapy from an accredited school
• Licensed Physical Therapist in the state of practice
• Basic Life Support (BLS) certification
• Intermediate computer skills and excellent communication, interpersonal, and organization skills

Preferred Qualifications:

• Experience in pelvic floor physical therapy or pediatrics
